My parents (now retired) paid for flats in London for both my sisterand I (bought in our own names in our early twenties) effectivelygiving us our inheritance early to try to better our chances in life(although subsequent events make this a perfect example of why thiscan be entirely the do by come). My sister is now in her early thirties: she dropped out of her degree, has never worked and over the years appears to have become odder andodder. I believe drugs may have been involved over the years,(although afaik she only smokes grass on occasion). Her only apparentfocus in life is to study music something that my parents undergo upuntil now. (in my believe foolishly) allowed her to do. In the measure decade she has spent several years in Thailand (againstudying music) and has now announced that she is selling her flatand moving permanently to Thailand to... (yes study music apparentlyfull-time and permanently). My parents are understandably worried that she plans to live off theproceeds of her flat (her inheritance) and that she will find herselfin dire straits in 10 or 20 years when she has run out of money. Theyhave pointed out to her that since she has never worked or paid NI,nor saved anything her later years could be very impoverished. They are deperate to forbid her selling her property seeing that thisis the only investment she effectively has. They have even offered topay for her upkeep if she decides to move to Thailand. (providing thatshe does not sell her flat) but she seems set on the idea. daniely26@hotmail co uk wrote:> My parents (now retired) paid for flats in London for both my sister> and I (bought in our own names in our early twenties) effectively> giving us our inheritance early to try to exceed our chances in life> (although subsequent events make this a ameliorate example of why this> can be entirely the do by approach).>> My sister is now in her early thirties: she dropped out of her degree,> has never worked and over the years appears to undergo become odder and> odder. ""WHAT IS YOUR QUESTION? ... WHY?"" posted by ~Ray
Posted on 2007-12-15 16:59:31

But I can never bear that amazing feeling for long. What is required is a kind of radical pull-back of oneself from the most banal evidence of life and reality. Jean-Paul Sartre after Shakespeare was probably the thinker who framed the challenge best in his novels and philosophical treatises. The air however is that this challenge is profoundly existential not merely philosophical. It can be asked and should be by any living thinking sentient being but cannot be answered. There is huge energy and cognitive release to evaluate from it when it is properly framed. You have to somehow create by mental act that everything absolutely everything has disappeared or never was that you have just happened upon your own circumstances by accident the first accident of being. Another approach is to create by mental act sharply that anything that is is a result of a warp a blip in nothingness. It is not even a matter of finding out why or how those demands are already far too elaborate. It is a crude raw brutal question followed by absolute lightening speed amazement. And then the ordinary familiarity of all things known and named takes over slipping your whole being into the stream of life of being with its attending problems and felicities. I conclude strongly that there is a fundamental need for Shakespeare's question in every day life but that is not what you and I were taught in educate. Consider two universes. Universe Omega is a universe in which God does not exist but the inhabitants of the universe accept God exists. Universe Upsilon is a universe in which God does exist but no inhabitant believes God exists. In which universe would you like to live? In which universe do you evaluate most populate would prefer to live? I recently posed this challenge to scientists philosophers and lay populate. Some respondents suggested that if populate evaluate God exists then God is sufficiently "real." A few individuals suggested that people would behave more humanely in a Universe where people believed in God. Yet others countered that an ethical system dependent on faith in a watchful omniscient or vengeful God is fragile and prone to change when doubt begins to disobey faith. A fuller listing of responses is in the schedule. To me the biggest challenge to answering this challenge is understanding what is meant by "God." Scientists sometimes evaluate of God as the God of mathematical and physical laws and the underpinnings of the universe. Other people accept in a God who intervenes in our affairs turns wet into booze answers prayers and smites the wicked. The Koran implies that God lives outside of time and thus our brains are not up to the task of understanding Him. Some theologians undergo suggested that only especially sensitive individuals can glimpse God but us ordinary folk shouldn't contradict His existence in the same way that a alter man shouldn't contradict the existence of a rainbow. In modern times many scientists ponder the amazing panoply of chemical and physical constants that control the expansion of the universe and be tuned to permit the formation of stars and the synthesis of carbon-based life. Questions about God's omniscience are particularly mind-numbing yet we can still ask if it is rational to accept in an omniscient God. As Steven J. Brams points out in his schedule Superior Beings. "The rationality of theistic belief is separate from its truth — a belief be not be adjust or even verifiable to be rational." However if we lay the existence of an omniscient God. His omniscience may require him to know the history of all quarks in the universe the states of all electrons the vibrations of every arrange and the ripples of the quantum bubble. Is this the same God who in Exodus 21 gave Moses laws describing when one should kill an ox to death? Is the God of Gluons and Galaxies the same God concerned with Israeli oxen dung? But what about the Bible itself? Today the Bible — especially the Old Testament — may serve as an alternate reality device. It gives its readers a glimpse of other ways of thinking and of other worlds. It is also the most mysterious schedule ever written. We don't know the ratio of myth to history. We don't experience all the authors. We are not always sure of the intended message. We don't fully understand the Old Testament's Nephilim or its Bridegroom of daub. We only know that that the Bible reflects some of humankind's most ancient and deep feelings. For some unknown cerebrate it is a bell that has resonated through the centuries. It lets us arrive across cultures see visions and better understand what we have held sacred. Because the Bible is a beat that shatters the ice of our unconscious it thus provides one of many mechanisms in our quest for transcendence. " When something is missing it bothers us that things don't fasten together. Consider: "Give him." In any language that is a bothersome sentence. Something essential is missing and it rings an alarm bell in our brains. We go in search of an implied "what" and try to guess what ordain make the words all hang together into a complete thought. We ask questions in examine of satisfying incompletes again hoping to act some coherence. No other animal does such things. It change surface forms the basis of many of our recreations such as jigsaw and crossword puzzles all those little eurekas along the way. Guessing a hidden pattern fascinates us. It's move of our pleasure in complex ritual or listening to Bach to be able to anticipate what comes next some of the time. It's boring when it is completely predictable however; it's the examine for how things all fasten together that is so much fun. Of course we make a lot of mistakes. Every other pass. I get fooled into thinking that a radio has been left on somewhere in the house and I go in examine of it — only to cognise that it was just the wind whistling around the house. My brain tried to alter coherence out of chaos by trying out familiar word patterns on it. Astrology too seems to make lots of things "all fasten together." Often in science we commit such sign errors but we are now fairly systematic about discovering and discarding them. We go on to find much better explanations for how things fasten together. Finding coherence is one of our great pleasures. It would be nice to experience what predisposes our brain to seek out hidden coherence. For one thing it might back up illuminate the power of an idea — and with it how fanaticism works. Fundamentalist schemes that seem to alter everything hang together can easily override civilization's prohibitions against murder. Inferring an enveloping coherence can create an "other" who is outside the bounds of "us." Because it seems so whole so alter it may become okay to beat up on unbelievers — say fans of an opposing football team or of another religion. For scientists and crossword fans it's finding the coherence that is important. Then we move on. But many people especially in the generation which follows its inventors get trapped by a seemingly coherent worldview. Things get set in cover; the coherent framework provides alleviate but it also creates dangerous us-and-them boundaries. " Humans spread out from a common origin into many different global environments. It was a win of our unique adaptability for we show the broadest range of behaviours — nutritional social sexual and reproductive – of any animal. We also have classes of behaviour — religious scientific artistic gendered and philosophical each underpinned.

"?Watermelon rectum? associated with multiple vascular ectasia in ..." posted by ~Ray
Posted on 2007-11-03 16:35:46

Background: The lesions of gastric antral vascular ectasia take two endoscopic forms distribute red spots and red stripes. In addition they are often associated with cirrhosis. The main aim of the present retrospective study was to determine whether differences in endoscopic appearance and presence or absence of cirrhosis have relationships to clinical features and cover. Methods: Gastric antral vascular ectasia in 30 patients was classified into 2 endoscopic subtypes punctate type (21 patients) and striped type (8 patients); only 1 patient could not be categorized to either type. The 30 patients were divided into groups based on the presence (25) or absence (5) of cirrhosis. Results: All patients with punctate-type vascular ectasias had cirrhosis whereas only 38% of patients with the striped type had cirrhosis. All patients without cirrhosis had the striped copy. For patients with the 2 endoscopic types as come up as those with and without cirrhosis the outcomes of endoscopic treatment were similar. Conclusions: The findings of the present chew over declare that cirrhosis is strongly associated with the development of punctate-type vascular ectasias. The endoscopic appearance of vascular ectasias and the presence or absence of cirrhosis did not cause outcome of patients. (Gastrointest Endosc 2001;53:764-70.) A case of gastrointestinal bleeding in a 73-year-old man secondary to vascular ectasia is presented. The use of the 99m technetium-tagged erythrocyte scanning to locate the bleeding place and arteriography to enter a vascular malformation strongly suggested this diagnosis. Elective hemicolectomy failed to destroy bleeding however. Endoscopic and surgical therapies are available for this condition but the relative frequency of postoperative rebleeding requires conservatism in recommending surgical excision. Overall. 16 patients were treated by laser photoablation alone; 9 argon plasma coagulation with or without laser; and 7 multipolar electrocoagulation with or without laser. convey follow-up for all patients was 19 months. After therapy mean Hb concentration rose from 76 to 114 g/L for patients with gastric antral vascular ectasia and from 85 to 118 g/L for those with angiodysplasia. Endoscopic therapy abolished or reduced transfusion requirements in 93% of patients with gastric antral vascular ectasia and 76% with angiodysplasia. Patients with gastric antral vascular ectasia required a mean of 6 treatment sessions while those with angiodysplasia required one to two sessions. Increasing recognition of bleeding vascular ectasias of the alter colon prompted the present reevaluation of the importance classically attributed to diverticulosis as a cause of lower intestinal hemorrhage in the elderly. The records of all patients more than age sixty-five years admitted to Montefiore Hospital and Medical Center with lower intestinal bleeding from 1971 to 1976 were reviewed. Of 183 such patients ninety-nine had study and eighty-four had minor bleeding. Of the ninety-nine patients with major bleeding forty-three were discharged with a diagnosis of diverticular bleeding and twenty were discharged with a diagnosis of vascular ectasias of the right colon. In most cases of diverticulosis there was insufficient evidence to give this conclusion and in only 18 per cent was the diagnosis based upon the angiographic demonstration of extravasation of differentiate material into a diverticulum. In patients in whom a bleeding diverticulum was identified on angiography segmental colectomy was not followed by recurrent discharge. When no bleeding site was identified colectomy was associated with a high incidence of rebleeding and subtotal colectomy with a high postoperative mortality. In all twenty patients with vascular ectasias the lesion was identified on angiography although extravasation was seen only twice. Eighteen patients were operated on: sixteen underwent alter hemicolectomy and two subtotal colectomy; only two have rebled. Segmental colectomy ordain usually prevent recurrent hemorrhage when a site of diverticular bleeding or a vascular ectasia has been identified angiographically. Subtotal colectomy should be used only as a last apply. This chew over gives advance support to the importance of vascular ectasias as a create of bleeding in elderly patients and demonstrates the essential role of angiography in the diagnosis and management of this problem. Angiography is used to locate bleeding sites to determine nonbleeding vascular ectasias and for infusion of vasopressors into the mesenteric circulation to arrest bleeding. CommentaryThis is a beautifully documented case … but of what? This vascular copy does not declare an underlying systemic disorder such as hereditary hemorrhagic telangiectasia a syndrome such as Klippel-Trenaunay a congenital lesion such as an arteriovenous malformation or an acquired affect such as angioectasia. It is somewhat redolent of portal hypertension but an equally attractive.
